For Self and Plan Managed NDIS Clients in Capacity Building under “Increased Social and Community Participation”.

  • Innovative Community Participation. (Item Number: 09_008_0116_6_3).
    Building capacity to seek assistance within the community without formal support including:
    – navigating various programs and systems to effectively engage in community activities,
    – understanding the impact of self-esteem, confidence, independence, and disability,
    – developing adaptive social skills including recognising social norms and expectations,
    – fostering self-regulation and emotional intelligence,
    – recognising and connecting personal passions, skills and interests,
    – promoting self-care and introspection, and
    – acknowledging the importance of relationships, social involvement, and community engagement in sustaining mental and emotional health.

Individual sessions are at a cost of: $130 for up to 60 minutes (50 minutes client contact).

For Self Managed NDIS Clients in Capacity Building under “Increased Social and Community Participation”

  • Life Transition Planning Incl. Mentoring Peer Support and Individual Skill Development. (Item Number 09_006_0106_6_3).
    Life Transition Planning is designed to aid NDIS participants with tailored mentoring, peer support, and skill enhancement opportunities. It encompasses one-on-one guidance focusing on personal growth, vocational and educational aspirations, as well as community engagement. Mentors and peers proficient in disability support contribute deeply to the individual’s personalised development plan, ensuring relevance and efficacy. This comprehensive service can encompass aspects from daily living skills to complex decision-making processes.

    These sessions can include:
    – stress and anxiety management,
    – anger management,

    – time management
    – emotional regulation,

    – identifying, accessing and navigating tools and resources to achieve NDIS goals,
    – developing Resilience and
    – improving Social Skills and more

Individual sessions are at a cost of: $130 for up to 60 minutes (50 minutes client contact).

Email support and some phone support is also available in between sessions.

Phone calls, emails or any correspondence longer than 10 minutes will be charged out in 15-minute increments.

Cancellation Fees = 100% of the full fee applies to all appointments cancelled with less than 24 hours notice UNLESS I can fill your timeslot.

Online Sessions

Some of my sessions can be conducted online via Zoom or phone for those that cannot get to the office here in Langwarrin.

This has proven successful with some of my clients who are highly anxious and cannot leave the house and for others who have transportation issues.

Also of great benefit is the fact that Zoom sessions can be recorded for future reference, eliminating the need for you to take notes during the session.

Other advantages of Zoom sessions are that they are great for our visual learners as I can physically show you how to practise different strategies and we can do that together in real time.  We can also have more than one family member join in on the session giving everyone the same information at the same time.  A great tool for family sessions.

Individual and/or small group (up to 4 participants) sessions available.

Suitable for:

  1. All ages (10yrs old and above)
    In times of illness.
  2. Teens
    A great way for our teens to reach out and connect in a way they feel comfortable and in a small group setting or 1:1.
  3. Adults (partners/parents or 1:1)
    Those with high anxiety and cannot leave the house, have transport issues or parents who do not have childcare options.

Initial sessions are followed up with a comprehensive email outlining session notes and relevant links.
Some phone and email support is also available in between following sessions.

Fees for online group sessions and webinars vary according to length and number of participants.

Once you are ready to access supports and you have your Plan in place, contact me for a free confidential chat where we can discuss your needs, all costings and make an appointment to finalise your Service Agreement.  (This Service Agreement is a mutual agreement made between client and provider safeguarding both parties.  It includes items such as: the supports to be covered, at what location, at what price, length of the agreement, responsibilities of both parties and notice required for cancellations etc.  Standard notice to cancel services is 2 weeks notice to be agreed upon by both parties.)
